Copies have been observed with a copy of the Arkham House copyright cancel but without the Cook 1928 copyright underneath.
Roy A. Squires (in Jaffery, below) states that the binding alone does not denote a forgery as legitimate copies of unbound sheets may have been eventually bound similarly.
The first recorded copy of the forgery was seen in London in June 1966. [Currey]
