Notes: Todd Lockwood used Kickstarter to raise the money between 2022-10-06 and 2022-11-15 to publish this edition. He distributed at least 807 copies of the book, along with other material depending on the donation. There were two versions, the trade edition and a special edition bound in leather and in a slipcase. The lowest price for a book was $60, the most expensive package was $400. The ISBN shown is for the special edition, the ISBN for the Grim Oak Press version is identical to that for the trade edition. Grim Oak Press later published the same material, and had no control over content. The only difference between the trade edition and the Grim Oak edition is the dust jacket. The "Untitled Comments" are by friends of Todd Lockwood commenting on his life.
