Chalker/Owings states that the actual number printed is 371 and that only 150 or so were ever bound with the remaining sheets discarded. They continue to state that the announced hardcover was never done.
Chalker/Owings also states that all Mirage copies are numbered 188 of 450 which is the number of Chalker's original copy from which this edition was done.
