Information from examination copy of book: Identical to regular edition except that cover is glossy 40lb cardstock instead of bookpaper and the presence on the title page of a rubber-stamped notice "THIS IS NO. _________ OF A SPECIAL ISSUE OF ___________COPIES SIGNED BY THE AUTHOR. The number and limitation are handwritten in and the author has signed under the title on the title page. The book and checklist were issued in a plain 5" x 7-1/2" manila envelope.
