- Library of Congress (LCCN, below) reports of its Copy 2, the copyright deposit copy, "No. 119 (July 1939) of Monthly letter of Limited Editions Club laid in." The book is LEC number 112.
- Library of Congress (LCCN, below) reports, presumably from the title page, "with a preface by Mr. Shaw, and with wood engravings by John Farleigh". WorldCat record OCLC 1684867 quotes the Colophon, "The illustrations for this edition were engraved in wood by John Fairleigh[sic] with the advice of Mr. Shaw. The edition was designed [...]". "Farleigh" is the correct spelling.
- The 1921 "Preface: The Infidel Half Century" may be considered integral to the work; that is, to a collection comprising that essay and five plays. The 1939 "Introduction" listed here represents the new preface Shaw may have composed for this edition (probably containing "The Infidel Half Century" too), perhaps concerning its illustration by Farleigh in consultation with Shaw.
